Phyllis Johnston Burquist, age 64, a resident of Theodore, AL passed away on Monday, August 17, 2015. She is survived by her husband, John Rudy Burquist, children, Timothy Burquist, Michael Burquist, Johnny Burquist and Shannon Bates, 7 grandchildren, other relatives and many friends. Funeral service will be held at Serenity Funeral Home on Friday, August 21, 2015 at 12 noon with visitation starting at 11 am until service time. Interment will be in Serenity Memorial Gardens.
